How To Pay
There are a number of convenient ways to pay for a test. Most testers simply pay as an out-of-pocket expense due to affordability. ZRT accepts major credit cards, VISA, Mastercard, Discover and American Express for your convenience.
Many find that an excellent way to help underwrite the testing fees is by utilizing pre-tax dollars accrued in medical or flex spending accounts, referred to as Flexible Spending Accounts (FSA) or Health Savings Accounts (HSA), that are often made available through employers.
Additionally, a number of insurance companies will provide some form of reimbursement. Please call your insurance carrier to understand your plan coverage before testing. Please note:
- ZRT does require either payment or insurance information for laboratory services at the time a specimen is received.
- If full payment is received, ZRT will provide an itemized receipt upon request so that you may submit your own insurance claim.
If you intend to submit insurance information, currently ZRT will ONLY bill the companies listed below:
Contracted: PersonalCare Insurance of Illinois First Health CCN Network Cook Group Health Plan
ZRT Laboratory will also bill the following as a non-participating provider, as a courtesy ONLY: Cigna Humana Medical Mutual TriCare Medicare Private Fee for Service Medicare Part B*
*Saliva testing is no longer a covered service for beneficiaries with Medicare Part B coverage. An excellent alternative however, is our dried blood spot test. We can perform the majority of the same tests in blood spot and there would be no out of pocket expenses for Medicare patients. Our dried blood spot test is appropriate for individuals who have a persistently dry mouth and cannot collect enough saliva. This is particularly problematic with aging but is made worse by many of the prescription drugs taken by the aging population.
If saliva testing is preferred, an Advanced Beneficiary Notice must be completed and returned to the lab, prior to sample collection. Please review the following instructions regarding the Advanced Beneficiary Notice.
The lab does not accept responsibility for disputed, denied, or unpaid insurance claims. In the event the insurance company does not cover the entire costs of testing, the balance due is your responsibility. For more information, see our FAQ's section (General tab).
Check with your employer's benefits administrator to see if you participate in a Flexible Spending Account (Flex Plan) or Health Savings account (HSA). These plans allow you to set aside funds from your wages before taxes, which you can use to pay for medical expenses that are not covered by your health plan. These could include ZRT testing. Although the expense is out-of-pocket, you have full control of your health care decisions and save pre-tax dollars.
Other payment option considerations:
If you received a kit from your health care provider, please consult them first to determine if your payment needs to be made to your clinic or ZRT Laboratory. Often times, working with a provider in the ZRT Network will result in discounted pricing availability. Click here to find a ZRT provider in your area.
