Step 2: Complete the Become a ZRT Provider Form

Partnering with ZRT Laboratory opens the door to a world of opportunities for enhancing patient care. Setting up a free account is simple and straightforward. To complete the application, please have the following information ready:

Ordering Health Care Provider's Name and Credentials
- License #
- NPI # (if applicable)
Business Name and Location
Contact Name, Email, and Phone number

Step 3: Start Testing with ZRT

After you submit your completed Become a ZRT Provider form, a member of our dedicated customer service team will activate your account. This will give you access to your myZRT portal, where you can effortlessly order kits, view reports, update your account information, and have access to our full range of services.
